VOLUNTEERS - PLEASE BE SURE TO CHECK ALL BUILDINGS WHERE YOU ARE APPLYING TO ATTEND, SO THAT YOU ONLY NEED TO COMPLETE ONE FORM TO VOLUNTEER FOR ANY BAS EVENT.  ONLY ONE VOLUNTEER FORM IS REQUIRED PER SCHOOL YEAR.  IT IS BEST TO COMPLETE THE FORM ON A LAPTOP OR DESKTOP COMPUTER TO ASSURE THAT A COMPLETE FORM IS RECEIVED.  IF YOU HAVE ALREADY COMPLETED THIS FORM ONCE DURING THIS SCHOOL YEAR (JULY 1-JUNE 30) - PLEASE DO NOT COMPLETE THIS FORM AGAIN!

Allow 3-5 business days for completion after submission of this form.  After requests are fully processed, approvals are placed on a master list and shared with the main office staff in each building.  You will be able to check your status with the building staff or your child's teacher after the 3-5 business days.  If you have questions regarding this form, please contact the Human Resources Department at humanresources@brightonk12.com.
