    Job Title: Student Support and Family Service Coordinator   

     

    Job Goal: To assist consortium schools in providing school-based mental health

     

    services and consultation, and assist in linking children and their families

     

    in need of services with the appropriate community agencies.

     

    Qualifications: Must possess School Counseling license/endorsement and/or Licensed

     

    Social Worker or equivalent. Experience working with high risk students  preferred.

     

    Major Duties and Responsibilities

     
    • Perform mental health risk assessment on students; collaborate with school/family/student to develop a treatment plan

    • Creating and/or updating district protocols for crisis management and follow-through and will include checklists, release of information forms, the intake and referral process, reentry to school for students, media releases, etc.

    • Develops protocol for districts to utilize when Student Support and Family Service Coordinator is unavailable

    • Hold regular meetings with consortium schools leadership, including building level assistance teams when possible

    • Serve as the district access point for community providers, educators and parents

    • Acts as liaison between families and community agencies and coordinates school services with those of the agency, when necessary

    • Case management and follow-up

    • Visit homes when appropriate

    • Provides in-service resources and helps coordinate training for staff on building level procedures for recognizing, intervening and referring mental health issues

    • Facilitates home and school communication

    • Facilitate reentry coordination with treatment agencies

    • Assist in the development of parent and community education opportunities

    • Compiles data on referrals, services provided, etc.

    • Maintain an office schedule within consortium schools

     

    The consortium of schools include the AGWSR, Aplington-Parkersburg, Dike-New Hartford, and Grundy Center Community School Districts.  All four districts are located along Highway 20 in the Cedar Valley area and are within 35 minutes of Cedar Falls and Waterloo.
